I can speak from the experience with my truck. There is almost no difference in engine sound other than the typically motor noise of higher RPMs.

While not an incredibly amount of deceleration, it is quite noticeable. It will definitely help reduce the wear on your stock truck brakes while towing. It will also help on steep downhills unladen as well.

It is not abrupt at all. It doesn't work until the conditions are satisfied (first post by GDE). It isn't an instant on type brake. It takes about a 1/4-1/2 second to full ramp up so to speak.
